It is critical that City Garden receive Free and Reduced Lunch forms from all families.
We must have all forms by October 10, so please make submitting this form a priority. We invite families to submit their forms digitally or fill out a paper form and return it by mail.
A. If you wish to fill out a paper form by mail, please click below to fill out a two-question, online form notifying our team to mail you the necessary paperwork.
B. If you wish you to submit your form digitally, please follow the steps below.

2. Download the PDF to your computer and save it to your desktop.
3. Open the saved file on your desktop and fill out the form.
4. Save the file with your completed information.
